YOUR FIRST STEPS
Ready to join our practice as a patient? We just have a couple of things do take care of before we can schedule you. Click each of the buttons to complete each step.
First, fill out the required intake forms. Then download a records request form to complete and give to your provider. We will contact you to schedule your intake appointment once we receive your completed forms. Before your appointment go over our pre-appointment checklist. That's it!
WHAT TO EXPECT AT YOUR FIRST APPOINTMENT
During your first appointment, you are greeted by our friendly staff. Soon one of our providers will take vitals like weight, blood pressure, and heart rate. Then you answer a series of health questions. Our experts meet with you to discuss your previous medical history in-depth and do a thorough physical assessment, including neurological function indicators if needed. After gathering this information, our team will discuss next steps and make recommendations for your care plan. You may complete some labs in office or be sent with lab kits or lab orders to complete before your next appointment.
